At present due to high demand, we are unable to process private administrative work and are prioritising our NHS medical workload.

If you have any private administrative work you would like to have completed, please see further details below.

• HGV/taxi/driver/pilot licence medicals
• Holiday cancellation forms
• Private sick notes
• Re-employment medicals

You can take copies of your medical records to a private GP and they will be able to complete this for you. You can access your medical records through the NHS app which can be download through the Google Play Store or the Apple Store.

Alternatively you can request copies of your medical records at the practice using a SAR (Subject Access Request) form. It can take up to 30 days for us to process your request and we will send you copies of the same time period once, any further requests for the same time period will be deemed unreasonable requests under GDPR (General Data Protection Regulations).

• Insurance medicals/reports

We recommend that your insurance company send us a request through IGPR, this is an electronic way for them to quickly, safely access the information they need to complete your insurance medicals/reports.

Alternatively, they can request copies of your medical records using our 3^rd party SAR (Subject Access Request) form. When they contact us they will be informed how to do this.
